About our grants:

Grants are awarded based on need up to $1,000 per family per year. We will give out grants until our funds have been exhausted. As our funds grow, so will the number and amount of grants that will be awarded to those individuals showing financial need.

Eligibility:

Must show financial need and live in the state of Nebraska. All funds will be distributed directly to the medical provider to be applied towards a bill, not to the applicant.

Financial need of the family will be evaluated and documented through information provided on the application and by submission of a photocopy of the most recently filed Federal tax return (Internal Revenue Service 1040, 1040-A, or 1040-EZ). The following scale will be used to determine financial eligibility:

5 or more $125,000 or less

 

Ineligible Requests:

  • Medical bills already paid. Anything deemed non-medical will not be acceptable criteria to submit a grant request.
  • Medicaid, Medicare, SCHIP (which may be called various names by each state), HIS or other state or federally subsidized health insurance programs given to those without insurance or with low incomes.

If our foundation finds you eligible to have a medical expense paid for we will send an email to you confirming what funds will be provided and to which provider on your behalf.
